Work Schedule: Full-Time Days
Shift: Monday to Friday, 8 AM to 5 PM

Role Overview:
The Patient Care Advocate role is an outreach position within AdventHealth Medical Group, focusing on assessing patients requiring care for various medical conditions identified through data analysis. This position involves conducting chart reviews to determine patient eligibility for outreach. Patients will be contacted to arrange necessary appointments, and lab or imaging studies will be ordered to support their care. Coordination with the patient's primary care provider and staff is essential to ensure seamless care delivery. The Patient Care Advocate will also facilitate, assist, and implement quality improvement initiatives within the Physician Enterprise. Responsibilities may include data research, project plan development, monitoring quality improvement activities, individual case reviews, and providing education to team members, providers, and practitioners. Additionally, the clinical Patient Care Advocate will educate clinic staff and providers on Quality Measures and processes to meet these standards, supporting the ongoing development and maintenance of performance and clinical outcome standards. This role will also involve coordinating and implementing a clinical competency program for all non-provider clinical staff in outpatient clinic settings.

  • Manages responsibilities such as following up with patients to ensure attendance at appointments.
  • Exhibits strong attention to detail, a commitment to accuracy and excellence, and possesses outstanding communication and organizational skills.
  • Designs and implements programs or processes aimed at closing care gaps.
Qualifications:
The qualifications and experiences required for success in this role include:
  • A minimum of three years of clinical experience.
  • At least three years of experience with Electronic Medical Records (EMR).
  • An active LPN license in the state of Georgia.
