Job Summary

We are seeking a highly skilled Care Management Specialist to join our team at Regency Integrated Health Services. As a key member of our healthcare team, you will be responsible for coordinating the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments.

Key Responsibilities
  • RAI Process Coordination: Ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payer program requirements.
  • Assessment Accuracy: Ensure assessments accurately reflect the physical, mental, and psychosocial status of each resident.
  • Documentation: Ensure appropriate documentation to report and support services provided and assessment accuracy.
  • Interdisciplinary Team Collaboration: Communicate effectively with other members of the interdisciplinary team.
  • Policy and Procedure Adherence: Follow all RIHS policies and procedures.
Requirements
  • Education: Must be a graduate of an accredited school of nursing with current R.N. or L.V.N. licensure.
  • Experience: Minimum of two years health care experience, with experience in MDS completion, reimbursement, clinical resource utilization, and/or case management highly desirable.
  • Skills: Analytical reasoning, logical reasoning, problem-solving, time management, organizational skills, research skills, and excellent verbal and written communication and presentation skills.
