The University of California is seeking a highly skilled Pharmacist II - Clinical Operations to join our team at Parnassus Heights. As a key member of our Department of Pharmaceutical Services, you will play a critical role in providing high-quality, cost-effective patient care that promotes health.
Key Responsibilities:
- Evaluate and manage drug regimens for appropriateness of choice
- Teach patients and their caregivers how to use drugs in the safest and most effective way
- Help patients and their caregivers take an active role in managing their drug therapy
- Advocate for patients the safe and effective use of drugs to other health care decision makers
- Monitor and evaluate patients for appropriate therapeutic outcomes
- Review individual patients' records to ensure that drug therapy is appropriate
- Develop pharmaceutical care plans
- Disseminate drug information to health care professionals
- Participate in patient care conferences
- Participate in the design and completion of patient drug use reviews, audits and other drug-related studies and projects
Requirements:
- Must have graduated from an accredited school of pharmacy and possess a Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy
- Must possess a current license in good standing to practice pharmacy in the State of California
- Current certification for Basic Life Support (BLS) from the American Heart Association
