Job Title: Readmission Coordinator RN

UNC Health Care is seeking a highly skilled and compassionate Readmission Coordinator RN to join our team. As a key member of our Care Management department, you will play a vital role in identifying patients at high risk for readmission and developing strategies to prevent unnecessary hospital readmissions.

Key Responsibilities:
  • Conduct daily reviews of readmissions and high-risk patients to identify opportunities for intervention.
  • Collaborate with multidisciplinary teams, including physicians, social workers, and external agencies, to develop and implement effective discharge plans.
  • Facilitate medication education and adherence visits, as well as coordinate follow-up appointments and track compliance.
  • Provide social determinants of health reviews and connect patients with community resources to meet their needs.
  • Work closely with nursing leaders and quality teams to track and trend key performance indicators, including readmission rates and ED readmission avoidance.
Requirements:
  • Graduate of an accredited nursing program.
  • Current NC license as a Registered Nurse.
  • Minimum of two years of experience in nursing, including care management or quality improvement.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
