Key Responsibilities:
  • Conduct interviews and evaluations of all residents prior to the initial Care Plan Conference; document findings in the medical record, develop personalized recreation plans based on assessments, and participate in Interdisciplinary Care Plan meetings.
  • Revise assessments and plans as necessary to comply with state or federal regulations.
  • Create monthly recreational program calendars that address the needs of facility residents.
  • Communicate facility programs effectively to residents, staff, families, and volunteers.
  • Oversee the facility's Volunteer Program.
  • Maintain departmental records that document services provided and resident progress towards established goals.
  • Collaborate with social services to assist residents in organizing and further developing a Resident's Council.
  • Assign tasks and establish priorities for team members.
  • Participate as a member of the Quality Assurance and Assessment committee.
Qualifications:
  • Must hold a current license or registration in the state of practice as an Activities Director, Social Worker, Occupational Therapist (OT), or Occupational Therapy Assistant (OTA), as required by state regulations.
  • At least two (2) years of experience in social or recreational programming within the last five (5) years.
  • A minimum of one (1) year of full-time experience in a healthcare activities program.
  • Certification as a Therapeutic Recreation Specialist is preferred.
