Sign-On Bonus: We are excited to offer a sign-on bonus of $1500 upon hire, with additional bonuses of $1,000 after the first month and $500 after six months of employment.

Position Overview:

The Client Care Specialist is responsible for performing crisis intervention and stabilization tasks. This role involves assisting clients and their guardians in navigating community resources and completing essential documentation for referrals, admissions, discharges, and out-of-home placements. The specialist serves as a key support for the clinical team and engages in care coordination, ensuring compliance with treatment regulations and authorizations for residential placements. This position reports to a Senior Team Lead.

Work Schedule:

This role follows a Monday to Friday schedule, with flexibility to work during clinic hours.

Compensation: $20.00 per hour

Key Responsibilities:

  • Review and request necessary documentation to determine the appropriate level of care for clients.
  • Assess eligibility for services and benefits.
  • Develop individualized service plans based on client assessments.
  • Maintain communication with utilization managers and out-of-home placements to ensure proper documentation and continuity of care.
  • Coordinate care by collaborating with the client’s care team to evaluate the effectiveness of interventions and adjust service plans as needed.
  • Monitor clients' mental health status and treatment responses through regular face-to-face interactions.
  • Provide crisis intervention and develop safety plans, conducting group sessions as required.
  • Facilitate transitions to appropriate care levels upon treatment completion.
  • Offer information and support to clients and their families regarding community resources.
  • Assist clients and families in scheduling appointments and arranging transportation for services.
  • Maintain thorough and accurate documentation of client interactions and treatment progress.
  • Ensure close collaboration with primary care providers, stakeholders, and funding sources for all clients.
Qualifications:
  • Must be at least 22 years of age.
  • A combination of education and experience totaling three years is acceptable.
  • A Bachelor’s degree with at least one year of full-time experience in behavioral health is preferred.
  • Valid driver's license and a clean driving record are required.
  • Fingerprint clearance from the Arizona Department of Public Safety is necessary or must be obtained within eight weeks of hire.
