Southwest Behavioral Health Services is seeking a dedicated and empathetic Care Coordinator who can help us fulfill our mission of delivering compassionate care to enhance lives and improve communities.
Key Responsibilities- Perform crisis stabilization functions to ensure the well-being of consumers.
- Assist consumers and/or guardians in accessing resources within the community.
- Complete necessary paperwork for referrals, coordination, admission, discharge, and out-of-home placements.
- Serve as central support for the clinical team.
- Perform care coordination functions, including clinical care coordination between resources within and outside the agency.
- Monitor compliance with court-ordered treatment rules and regulations.
- Authorize residential or out-of-home placements as needed.
- Minimum age requirement of 22 years.
- A combination of experience and education totaling 3 years is acceptable.
- Bachelor's degree with one year of full-time behavioral health experience.
- Valid AZ driver's license and motor vehicle clearance.
- Fingerprint clearance issued by the Arizona Department of Public Safety or eligibility to receive within eight weeks of hire.
- 3.3 weeks of PTO in the first year of employment, with increased accruals after continued service.
- 10 paid holidays.
