At Day City Hospice, we are dedicated to providing exceptional patient-centered care to individuals in their most vulnerable time. As a Hospice Care Specialist, you will play a vital role in delivering compassionate and personalized care to our patients and their families.
Responsibilities
- Provide personal care needs, including bathing, grooming, and dressing, as assigned.
- Meet safety needs of patients and use equipment safely and properly.
- Assist with meal planning and preparation, including shopping and feeding patients as assigned.
- Take and record vital signs, including temperature, pulse, respiration, and blood pressure, as ordered.
- Provide proper care and observation of patient skin to prevent breakdown of tissue over bony prominences.
- Report on patient condition and significant changes to the Case Manager.
- Provide respite for patients, families, and caregivers as needed.
Requirements
- At least 16 years of age.
- Certified by the state as a nurse aide.
- Maintains a current CPR Certification.
- Listed on the state aide registry with a clear record.
- Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
