Job Description

Overview

At Day City Hospice, we are dedicated to providing exceptional patient-centered care to individuals in their most vulnerable time. As a Hospice Care Specialist, you will play a vital role in delivering compassionate and personalized care to our patients and their families.

Responsibilities

  • Provide personal care needs, including bathing, grooming, and dressing, as assigned.
  • Meet safety needs of patients and use equipment safely and properly.
  • Assist with meal planning and preparation, including shopping and feeding patients as assigned.
  • Take and record vital signs, including temperature, pulse, respiration, and blood pressure, as ordered.
  • Provide proper care and observation of patient skin to prevent breakdown of tissue over bony prominences.
  • Report on patient condition and significant changes to the Case Manager.
  • Provide respite for patients, families, and caregivers as needed.

Requirements

  • At least 16 years of age.
  • Certified by the state as a nurse aide.
  • Maintains a current CPR Certification.
  • Listed on the state aide registry with a clear record.
  • Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
