POSITION SUMMARY

Reporting to the Director of Case Management, this role involves overseeing the daily functions and supervision of the RN Inpatient and Emergency Department Case Management Team across all NCH facilities. The focus is on delivering patient-centered care while striving to meet high standards of quality and patient satisfaction. This position requires leveraging performance management skills to achieve organizational goals and ensuring staff accountability.

KEY RESPONSIBILITIES

  • Implement organizational strategies for case management with an emphasis on enhancing patient experience.
  • Ensure adherence to regulatory standards and compliance.
  • Provide training and mentorship to RN case managers.
  • Conduct personnel functions including staffing, performance evaluations, and corrective actions.
  • Monitor productivity metrics and uphold standards.
  • Facilitate professional development opportunities for the Case Management Team.
  • Conduct regular team meetings to promote engagement in quality improvement initiatives.
  • Ensure active participation in multidisciplinary rounds to address care transitions and discharge planning.
  • Manage metrics related to length of stay, readmission prevention, and discharge planning.
  • Engage in weekly reviews of complex cases.
  • Oversee case management documentation to meet departmental guidelines.
  • Advocate for timely and effective discharge planning involving all relevant parties.
  • Participate in quality improvement initiatives and report concerns as necessary.
  • Collaborate with the Social Work Manager to ensure effective teamwork.
  • Work with the utilization management team to ensure compliance with regulatory requirements.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • BSN required; MSN preferred.
  • Minimum of 2 years in a supervisory or managerial role within hospital Case Management.
  • Current RN licensure in Florida or compact state.
  • BLS certification from a recognized provider.
  • Case Management Certification is preferred.
  • Proficient in Microsoft Office Suite including Word, Excel, and Outlook.
