POSITION SUMMARY

Under the guidance of the Director of Case Management, this role is responsible for overseeing the daily operations and management of the RN Inpatient and Emergency Department Case Management Team across all NCH facilities. The focus is on patient and family-centered care, ensuring that customer expectations for quality and satisfaction are met.

KEY RESPONSIBILITIES

  • Implement organizational strategies for case management with an emphasis on patient experience.
  • Ensure adherence to regulatory standards and compliance.
  • Provide training and mentorship to RN case managers.
  • Conduct personnel functions, including staffing, performance evaluations, and corrective actions.
  • Monitor productivity standards and ensure they are met.
  • Implement best practices in discipline and performance management.
  • Support staff through resource identification and utilization.
  • Encourage professional development within the Case Management Team.
  • Facilitate regular staff meetings to promote participation in quality improvement initiatives.
  • Ensure active staff involvement in multidisciplinary rounds for effective discharge planning.
  • Manage metrics related to length of stay, avoidable days, readmission prevention, and discharge planning.
  • Participate in weekly reviews of complex cases.
  • Record and report on indigent fund expenditures.
  • Implement monitoring techniques to ensure compliance with documentation standards.
  • Advocate for timely and effective discharge planning involving all stakeholders.
  • Report quality concerns promptly to relevant parties.
  • Assist in developing and overseeing care management policies and procedures.
  • Collaborate with the Social Work Manager to ensure effective teamwork.
  • Work with the utilization management team to ensure timely regulatory form submissions and appropriate care levels.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred.
  • A minimum of 2 years in a supervisory or managerial role within hospital case management is required.
  • Current Registered Nurse (RN) license in Florida or appropriate compact licensure.
  • Basic Life Support (BLS) certification from a recognized provider.
  • Case Management Certification is preferred.
  • Proficient in Microsoft Office Suite, including Word, Excel, and Outlook.
