POSITION SUMMARY

Reporting to the Director of Case Management, this role is responsible for overseeing the daily functions and management of the RN Inpatient and Emergency Department Case Management Team across all NCH facilities. The position emphasizes patient and family-centered care as the utmost priority while striving to fulfill customer expectations regarding quality and satisfaction.

KEY RESPONSIBILITIES

Essential duties include, but are not limited to:

  • Implementing organizational strategies focused on enhancing patient experiences.
  • Ensuring adherence to regulatory compliance standards.
  • Training and mentoring RN case managers.
  • Conducting personnel management tasks, including staffing and performance evaluations.
  • Monitoring productivity metrics and ensuring standards are met.
  • Promoting best practices in discipline and performance management.
  • Supporting staff through resource identification and utilization.
  • Encouraging professional growth within the Case Management Team.
  • Facilitating regular team meetings to foster collaboration and quality improvement.
  • Ensuring participation in multidisciplinary rounds to address care transitions and discharge planning challenges.
  • Managing metrics related to length of stay, readmission prevention, and discharge planning.
  • Engaging in weekly reviews of complex cases.
  • Documenting and reporting on indigent fund expenditures.
  • Implementing monitoring techniques to ensure compliance with documentation standards.
  • Facilitating timely and effective discharge planning involving all relevant parties.
  • Advocating for timely care and addressing any delays.
  • Participating in quality initiatives and reporting concerns to appropriate channels.
  • Assisting in the development and implementation of care management policies.
  • Collaborating with the Social Work Manager to ensure effective teamwork.
  • Working with the utilization management team to ensure timely completion of regulatory documentation and appropriate care levels.

EDUCATION AND EXPERIENCE REQUIREMENTS

Qualified candidates will possess:

  • A minimum of a Bachelor of Science in Nursing (BSN); a Master of Science in Nursing (MSN) is preferred.
  • At least two years of supervisory or managerial experience in hospital case management.
  • Current licensure as a Registered Nurse (RN) in Florida or appropriate compact licensure.
  • Basic Life Support (BLS) certification from a recognized provider.
  • Preferred certification in Case Management.
  • Proficiency in Microsoft Office applications, including Word, Excel, and Outlook.
