Care Manager RN

We are seeking a skilled RN Case Manager to join our care team at Providence. As a key member of our multidisciplinary team, you will work closely with patients, families, physicians, and other healthcare professionals to provide high-quality, patient-centered care.
Responsibilities- Coordinate care for an assigned panel of patients, working collaboratively with the care team to ensure seamless transitions and optimal outcomes.
- Develop and implement individualized care plans, taking into account patients' unique needs, preferences, and goals.
- Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange.
- Monitor patients' progress, identifying and addressing any barriers to care or potential complications.
- Collaborate with social workers, counselors, and other support staff to provide comprehensive care and address patients' social determinants of health.
- Graduation from an accredited school of nursing.
- Alaska Registered Nurse License upon hire.
- 2 years of professional nursing experience, with a focus on case management or a related field.
- Bachelor's Degree in Nursing.
- 2 years of case management experience in a clinic or outpatient setting.
