RN Care Manager

We are seeking a skilled RN Care Manager to join our team at Providence Home and Community Care. As a RN Care Manager, you will be responsible for coordinating patient care activities with other members of the healthcare team to promote patient comfort and assist in the achievement of a comfortable and peaceful death.
Key Responsibilities- Provide direct and indirect quality care for patients and families of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act.
- Manage a caseload of patients living within the service area of hospice.
- Coordinate patient care activities with other members of the healthcare team in a collegial relationship to promote patient comfort and assist in the achievement of a comfortable and peaceful death.
- Uphold the standards of confidentiality, attendance, and punctuality.
- Graduate of an accredited school of nursing.
- Alaska Registered Nurse License upon hire.
- Experience in Oncology, Hospice, Long Term Care, or Homecare is preferred.
