Care Manager RN

We are seeking a skilled Care Manager RN to join our team at Providence Alaska Medical Center. As a Care Manager RN, you will play a critical role in ensuring the seamless transition of patients through the healthcare system, providing exceptional care and support to our patients and their families.
Key Responsibilities- Partner with patients, physicians, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' medical, social, and emotional needs, identifying opportunities for improvement and developing strategies to address these needs.
- Collaborate with healthcare providers, community resources, and family members to coordinate care and ensure a smooth transition from hospital to home or other care settings.
- Advocate for patients' rights and interests, ensuring that their voices are heard and their needs are met.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Nursing degree or diploma
- Alaska Registered Nurse License
- 2 years of clinical nursing experience
- Bachelor's Degree in Nursing
- 3 years of clinical nursing experience
- 1 year of utilization management, review, and discharge planning experience
