Care Manager RN

We are seeking a skilled Care Manager RN to join our team at Providence Alaska Medical Center. As a Care Manager RN, you will play a critical role in ensuring the continuum of care for our patients, working closely with healthcare professionals, community resources, and families to provide coordinated admission and discharge planning.
Key Responsibilities- Partner with patients and physicians to assure access, services, and transition for the continuum of care.
- Work as a team member with healthcare professionals, community resources, liaisons, and families to provide coordinated admission and discharge planning.
- Advocate and support the case management process using the Physician-Centric Care Management Model as a guideline.
- Identify and achieve timely patient care needs with implementation of the plan of care that is appropriate, resourceful, and valuable.
- Effectively utilize tools that measure, monitor, and manage quality and stewardship of patient care resources.
- Nursing degree/diploma
- Alaska Registered Nurse License
- 2 years Clinical nursing experience
- Preferred qualifications: Bachelor's Degree in Nursing or higher, 3 years Clinical nursing experience, 1 year Utilization Management-review and Discharge-planning experience
