Case Manager Job Description

We are seeking a dedicated and compassionate Case Manager to join our team at Lifelong. As a Case Manager, you will work closely with clients to assess their needs, develop personalized care plans, and connect them with necessary resources.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' physical and emotional needs.
  • Develop and implement individualized care plans to improve clients' quality of life.
  • Collaborate with healthcare professionals, social workers, and other stakeholders to ensure seamless care coordination.
  • Provide emotional support and guidance to clients and their families.
  • Maintain accurate and up-to-date records of client progress and care plans.

Requirements:

  • 3-5 years of experience in social services or a related field.
  • Bachelor's degree in Social Work, Psychology, or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Commitment to providing exceptional customer service.
