We are seeking a skilled Nurse Care Coordinator to join our interdisciplinary team at UCHealth. In this role, you will work collaboratively with physicians, nurses, and other healthcare professionals to provide comprehensive care coordination services to patients and their families.

About the Role:

This is a full-time position working in the UCH A Care Management department. The successful candidate will have 2+ years of nursing experience and a strong background in patient advocacy, education, and care coordination. You will be responsible for assessing patients' needs, developing individualized care plans, and coordinating services to ensure seamless transitions across the continuum of care.

Key Responsibilities:
  • Collaborate with interdisciplinary teams to assess patients' needs and develop comprehensive care plans
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices
  • Coordinate services to ensure timely access to medical care, community resources, and social services
  • Communicate effectively with patients, families, and healthcare providers to ensure optimal outcomes
Requirements:
  • RN license in the state of Colorado
  • 2+ years of nursing experience in a similar role
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment with minimal supervision
