Job Title: Case Manager II

The Texas Department of Aging & Disability Services is seeking a highly qualified Case Manager II to join our team in the Interest List Management Unit.

Job Summary:

This is a journey-level case management position that involves developing and maintaining contact with individuals who are elderly or have disabilities to provide education on Medicaid Waiver programs and related service needs.

Key Responsibilities:
  • Develop and maintain contact with individuals who are elderly or have disabilities to provide education on Medicaid Waiver programs and related service needs.
  • Perform internal activities such as completing data entry in various databases, monitoring shared mailboxes, and communicating with the public over the phone regarding registration for waiver interest list programs.
  • Work requires adherence to contracts, policies, procedures, and guidelines to ensure consistency with Texas Health and Human Services Commission program requirements.
  • Attend work on a regular and predictable schedule in accordance with the agency leave policy.
  • Maintain and monitor high volumes of daily incoming waiver referrals.
  • Conduct outbound calls and perform data entry in the Community Services Interest List (CSIL) system, the HHS Enterprise Administrative Report and Tracking System (HEART), and SharePoint or other electronic programs and systems.
  • Provide back-up for the Medically Dependent Children Program (MDCP) money-follows-the-person requests, serve as a back-up answering the toll-free intake line for interest list calls, handle interest list inquiries, route calls to the appropriate area and take messages as necessary.
  • Maintain shared mailboxes as assigned.
  • Communicate on an intermediate level with others (internally and externally) to provide, exchange, or verify information, answer inquiries, address issues, or resolve problems or complaints.
  • Compose messages and answer moderately complex questions related to Medicaid waiver programs.
  • Prepare, edit, and proofread email correspondence, forms, or other documents such as letters and memos.
  • Maintain data entry for returned mail and program documents, to include retrieving or purging files according to the retention schedule, using manual or electronic systems.
Requirements:
  • Knowledge of community resources available to persons who are elderly or have disabilities.
  • Skill in use of the following office software applications (word, excel, and outlook).
  • Skill in use of standard office equipment (Personal computer, printers, copiers, fax and telephone).
  • Ability to perform data entry in an automated system while talking on the phone.
  • Ability to explain complex or difficult information in a pleasant and understandable manner.
  • Ability to organize workloads and set priorities to meet deadlines.
  • Ability to work under minimal supervision on difficult or complex tasks and maintain accurate files.
  • Ability to effectively manage time and priorities independently.
  • Ability to communicate with others (internally or externally) orally and in writing to provide, exchange, or verify information, answer inquiries, address issues, or resolve/report problems.
  • Ability to establish and maintain effective working relationships with co-workers, and managers in a fast-paced, deadline-oriented environment.
