Position Overview:
The Transplant Coordinator plays a crucial role in ensuring that patients receive outstanding care throughout their transplant journey. This position involves direct interaction with patients, attending daily interdisciplinary rounds, and addressing various clinical, psychosocial, and educational needs of post-transplant patients.
Key Responsibilities:
- Provide comprehensive education to patients and their families regarding post-transplant care, including follow-up appointments, laboratory tests, and coordination with the multidisciplinary team for specific patient needs.
- Facilitate communication with patients and caregivers about the recovery process and assist in discharge planning.
- Prepare written summaries of transplant education for documentation in the Electronic Medical Record, detailing the education provided and its outcomes.
- Conduct follow-up calls within 24-48 hours post-discharge to review education, discuss upcoming appointments, and address any concerns.
- Document inpatient care summaries to inform the post-transplant coordinator about care provided during hospitalization and any adjustments made to the care plan.
- Utilize clinical judgment based on current research and principles to address complex patient care challenges.
- Collaborate with medical professionals, nursing staff, pharmacy, and case management to meet the needs of patients and their families.
- Stay informed on the latest post-transplant research and apply evidence-based practices to patient care.
- Complete necessary regulatory documentation in the UNOS database and hospital electronic medical records.
- Review educational materials and collaborate with the department to implement changes in practice as needed.
Work Environment:
This role requires participation in an on-call rotation as part of a larger team dedicated to providing high-quality patient care.
Qualifications:
Education: A Bachelor of Science in Nursing (BSN) is required; a Master of Science in Nursing (MSN) is preferred.
Experience: A minimum of five years of nursing experience, with at least two years in transplant, critical care, or a chronic disease management setting.
Licensure: Current or compact RN licensure in North Carolina is required. Basic Life Support (BLS) certification is also required.
