Key Responsibilities:
  • Implement and coordinate the delivery of care in collaboration with physicians and care associates.
  • Assess, plan, implement, and evaluate resident care plans.
  • Provide high-level care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing resident overall condition and behavior.
  • Maintain ongoing communication with residents, families, and physicians regarding resident care.
Requirements:
  • High school diploma or equivalent (GED)
  • LPN License in applicable state
  • First Aid & CPR Certified
  • At least 1 year of related work experience
