Key Responsibilities
- This role is primarily field-based, focusing on care management to facilitate the coordination of services. This includes evaluating healthcare requirements, making referrals to healthcare providers, educating families about benefits and expanded services, and connecting them with community resources to address social determinants of health.
- The caseload will consist of 100 clients.
- Documentation will be maintained in an electronic health record system, requiring outreach notes, comprehensive assessments, and the development of individualized care plans for each client.
Required Qualifications
1. Proven experience in linking families and children to local resources for a variety of services, including employment, housing, independent living, justice, and foster care.
2. Experience in pediatric case management.
3. Background in foster care and/or adoption services.
4. Familiarity with health plan operations.
Educational Requirements:
1. A Bachelor's Degree is mandatory.
