Job Summary

We are seeking a skilled Clinical Social Worker to join our team at Corewell Health. As a Clinical Social Worker, you will work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. Your expertise will be essential in providing assessment, intervention, and support to patients and families, ensuring they receive the best possible care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized plans for care.
  • Provide counseling and support to patients and families, addressing issues related to mental health, substance abuse, and other psychosocial concerns.
  • Collaborate with healthcare teams to develop and implement discharge plans, ensuring a smooth transition of care for patients.
  • Provide education and resources to patients and families, promoting healthy behaviors and self-care.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • State of Michigan Social Work License (LMSW) or equivalent.
  • Experience working with patients and families with complex psychosocial needs.
  • Strong assessment and intervention skills, with ability to work effectively in a fast-paced environment.
  • Excellent communication and interpersonal skills, with ability to work collaboratively with healthcare teams.
