Job Summary

We are seeking a skilled Clinical Social Worker to join our team at Corewell Health. As a key member of our interdisciplinary team, you will work collaboratively with patients, families, and healthcare professionals to provide comprehensive psychosocial support and care coordination.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patient needs and develop effective care plans.
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Provide crisis intervention, counseling, and support to patients and families in need.
  • Develop and maintain knowledge of community resources to support patient needs.
  • Participate in departmental committees and projects to advance patient care and social work practice.
Requirements
  • Master's degree in social work from an accredited program.
  • State of Michigan Social Work License (LMSW) or equivalent.
  • Experience in individual, family assessment, crisis intervention, and discharge planning preferred.
