Job Summary

We are seeking a highly skilled Clinical Social Worker MSW to join our team at Corewell Health. As a Clinical Social Worker MSW, you will work collaboratively with our interdisciplinary team to assist patients and families with complex psychosocial needs.

Key Responsibilities
  • Conduct psychosocial assessments with patients and families to identify needs and develop plans for care.
  • Communicate effectively with healthcare teams and community agencies to ensure seamless transitions of care.
  • Provide consultation and resources to healthcare teams to support patient care.
  • Maintain current knowledge of community resources and federal/state regulations.
  • Participate in department or hospital committees and projects as assigned.
Requirements
  • Master's degree in social work.
  • State of Michigan Social Work License (LMSW expected within 5 years of employment).
  • Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
