Key Responsibilities

  • Assist patients managing chronic conditions
  • Engage in quality improvement initiatives
  • Oversee patient registries
  • Provide relevant data to address patient health requirements
  • Support practice personnel in managing designated patient populations
  • Enhance the overall patient experience
  • Identify and report discrepancies in data
  • Participate actively in team meetings

Qualifications

  • A minimum of 3 years of experience in a related field
  • Strong problem-solving skills with the ability to manage multiple tasks
  • Exceptional communication abilities
  • Certification as a Medical Assistant is preferred

Preferred Skills

  • Experience with Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
  • Fluency in both English and Spanish is preferred
  • Willingness to travel for in-person training sessions is preferred
