Job Summary

The Qualified Professional/Case Manager is a vital member of our team at Southeastern Integrated Care LLC, responsible for providing high-quality care to individuals in their homes and communities. This role requires a strong understanding of person-centered care principles and the ability to work collaboratively with clients, families, and healthcare professionals to develop and implement personalized care plans.

Key Responsibilities
  • Develop and implement person-centered care plans in collaboration with clients, families, and healthcare professionals.
  • Provide direct and indirect interventions to support clients in achieving their goals and improving their overall well-being.
  • Coordinate and facilitate access to community resources and services to meet clients' needs.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Document all interactions and interventions in a timely and accurate manner.
  • Participate in ongoing training and education to stay current with best practices in person-centered care.
Requirements
  • Bachelor's degree in a human services field or related field.
  • 2+ years of experience in a mental health or social services setting.
  • Strong understanding of person-centered care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid NC driver's license and reliable transportation.
Work Environment

This role requires the ability to work in a variety of settings, including clients' homes and communities. The work environment may involve exposure to cigarette smoke, domestic animals/pets, household pests, and other issues related to the domestic location in which service is delivered.

Physical Demands

The physical demands of this role include standing, walking, using hands to finger, handle, or feel objects, tools, or controls, talking and hearing, and tasting and smelling. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.
