Intensive In-Home Qualified Professional

We are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional/Case Worker to join our team at Southeastern Integrated Care LLC. As a key member of our care team, you will provide critical support to individuals with mental health and developmental disabilities, working closely with our clients, families, and healthcare professionals to develop and implement personalized care plans.
Key Responsibilities- Person-Centered Planning: Collaborate with clients and their support networks to create and update Person-Centered Plans, ensuring that their unique needs and goals are addressed.
- Case Management: Coordinate services and referrals, linking clients with community resources and ensuring seamless transitions between care settings.
- Advocacy and Support: Provide emotional support and advocacy for clients, promoting their autonomy and independence.
- Documentation and Reporting: Maintain accurate and timely records, submitting reports and documentation as required.
- Team Collaboration: Work closely with our multidisciplinary team, including healthcare professionals, social workers, and support staff, to ensure comprehensive care and support.
- Education and Experience: Bachelor's degree in a human services field or related field, with a minimum of 2 years of experience working with mental health and developmental disabilities populations.
- Skills and Abilities: Excellent communication and documentation skills, with the ability to work independently and as part of a team. Must possess a positive attitude, empathy, and a commitment to providing high-quality care.
This role requires working in clients' homes and communities, with occasional travel to our office. You must be able to lift up to 20 pounds and have a valid driver's license with personal vehicle insurance.
