Intensive In-Home Qualified Professional

We are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional/Case Worker to join our team at Southeastern Integrated Care LLC. As a key member of our staff, you will provide critical support to individuals with mental health needs, working closely with our team to develop and implement personalized care plans.
Key Responsibilities- Person-Centered Planning: Collaborate with individuals, families, and healthcare professionals to create and update comprehensive care plans that address unique needs and goals.
- Case Management: Coordinate services, linkages, and referrals to ensure seamless support and care for our clients.
- Advocacy and Support: Provide emotional support, advocacy, and education to individuals and their families, promoting empowerment and self-advocacy.
- Documentation and Reporting: Maintain accurate and timely records, reports, and documentation, adhering to regulatory requirements and company standards.
- Team Collaboration: Work closely with our multidisciplinary team to share knowledge, best practices, and resources, ensuring comprehensive care and support.
- Education and Experience: Bachelor's degree in a Human Services field or related field, with a minimum of 2 years of experience working with mental health populations.
- Skills and Abilities: Excellent communication, documentation, and interpersonal skills; ability to work independently and as part of a team; flexibility and adaptability in a fast-paced environment.
- Certifications and Licenses: Valid NC driver's license and personal vehicle insurance coverage; current license or certification in relevant field (if applicable).
This role requires working primarily in clients' homes and communities, with occasional office-based work. You will be exposed to various environments, including domestic settings, and may encounter challenging situations. As a representative of Southeastern Integrated Care LLC, you will embody our commitment to providing high-quality, person-centered care.
Physical DemandsThis position requires regular standing, walking, and use of hands to perform essential duties. You will also need to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
