Job Summary

The RN Care Coordinator at Dignity Health - Community Hospital of San Bernardino is responsible for overseeing the progression of care and discharge planning for identified patients requiring these services. This role involves promoting quality of care, cost-effective outcomes, and adherence to hospital policies, standards of practice, and Federal and State regulations.

Responsibilities

The RN Care Coordinator will perform the following duties:

Collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute service providers to progress care toward optimal outcomes.

Advocate for patients and families by identifying, valuing, and addressing patient choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions.

Enhance the patient experience by striving to meet individual health needs while promoting quality of care and cost-effective outcomes.

Qualifications

The ideal candidate will have a minimum of two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing. A California RN license, AHA BLS card, and ability to pass annual Inter-rater reliability test for Utilization Review product(s) used are also required. Additionally, the candidate should possess critical thinking and problem-solving skills, ability to apply clinical guidelines, and collaborate effectively with multiple stakeholders. Professional communication skills, knowledge of CMS standards and requirements, and proficiency in prioritizing work and delegating tasks are also essential.

Preferred Qualifications

A graduate of an accredited school of nursing (Bachelor's Degree in Nursing (BSN)) or related healthcare field, at least five years of nursing experience, and Certified Case Manager (CCM) Accredited Case Manager (ACM-RN) or UM Certification are preferred. Knowledge of managed care and payer environment is also beneficial.

Compensation

The salary range for this position is $51.98 - $67.83 per hour. Dignity Health is an equal opportunity/affirmative action employer.
