Key Responsibilities- Oversee the progression of care and discharge planning for identified patients
- Collaborate with utilization management, nursing, physicians, and ancillary departments to promote quality of care and cost-effective outcomes
- Advocate for patients and families, identifying and addressing barriers to care transitions
- Enhance the patient experience through effective communication and coordination
- Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing
- California RN license
- AHA BLS card
- Ability to pass annual Inter-rater reliability test for Utilization Review product(s) used
- Strong critical thinking and problem-solving skills
- Effective communication and collaboration skills
- Knowledge of CMS standards and requirements
- Graduate of an accredited school of nursing (Bachelor's Degree in Nursing (BSN)) or related healthcare field
- At least five years of nursing experience
- Certified Case Manager (CCM) Accredited Case Manager (ACM-RN) or UM Certification preferred
