Position Overview

The Director of Social Services at Windsor Health & Rehabilitation Center LLC plays a crucial role in overseeing the comprehensive management of the psychosocial needs of our residents. This position requires the ability to provide counseling or collaborate with psychiatric professionals to enhance patient well-being and progress.

Key Responsibilities:

The individual in this role is entrusted with the authority to:

  • Evaluate and revise policies and procedures to ensure social services are delivered in accordance with federal, state, and local regulations.
  • Conduct interviews, assessments, and develop tailored plans to address the unique needs of residents.
  • Analyze the social service program within the facility and provide constructive recommendations to the administration.

Core Duties Include:

  1. Conducting admission assessments for new residents.
  2. Interviewing new patients to gather essential information.
  3. Completing Minimum Data Set (MDS) sections as required.
  4. Finalizing ASCEND documentation.
  5. Creating and updating care plans as necessary.
  6. Providing ongoing support to residents, including conducting 72-hour meetings and documenting care plans.
  7. Investigating allegations of mistreatment and grievances thoroughly.
  8. Facilitating discharge planning to ensure appropriate living arrangements are established prior to patient discharge.
  9. Addressing financial concerns by meeting with patients and their responsible parties.
  10. Intervening in family crises on behalf of residents.
  11. Offering counseling for significant life events impacting patients.
  12. Submitting ASCEND documentation to relevant agencies upon admission.
  13. Assisting residents in obtaining level of care extensions.
  14. Collaborating with the Business Office Manager to support long-term care residents in securing benefit approvals for extended stays.

Qualifications:

  • A Master’s degree in Social Work (MSW) is required.
  • Minimum of 2 years of experience in a Long-Term Care environment.
  • In the absence of a Master’s degree, a Bachelor’s degree in Social Work with 5 years of relevant experience is acceptable.
  • Flexibility in scheduling is essential.
  • Strong communication skills with the ability to resolve complaints and address concerns effectively.
