Job Summary

The Home Care RN Intake Coordinator plays a vital role in ensuring seamless transitions of care from hospital to home health. As a key member of the Health First Home Care team, this individual will facilitate clinical referral reviews, care management coordination, and collaboration with referral sources to provide exceptional healthcare outcomes.

Key Responsibilities
  • Perform individualized clinical assessments of patient medical records to ensure safe transitions of care.
  • Communicate with referral sources to gather demographics, diagnoses, and clinical information to facilitate continuity of care.
  • Utilize clinical nursing skills and judgment to identify and verify appropriateness of patients for home health level of care.
  • Collaborate with the clinical team to coordinate home health referrals and ensure exceptional on-going communication and documentation.
  • Meet monthly financial targets in collaboration with the liaison team to enhance business development opportunities.
Requirements
  • Associate Degree in Nursing.
  • Minimum two (2) years of acute care experience as an RN.
  • Current, valid State of Florida RN license or endorsement.
  • American Heart Association Basic Life Support Healthcare Provider Card upon hire and maintain.
Preferred Qualifications
  • Bachelor Degree in Nursing.
  • Minimum one (1) year home health nursing or case management experience.
Physical Requirements

This role involves sitting or standing for long periods, occasional walking, bending, and stooping. It also requires light work that may include lifting or moving objects up to 20 pounds with or without assistance.
