Job Summary

The Health First Home Care Registered Nurse Intake Coordinator plays a vital role in ensuring seamless transitions of care from hospital to home health. This position requires a skilled and compassionate RN to facilitate clinical referrals, coordinate care management, and collaborate with referral sources to deliver exceptional patient outcomes.

Key Responsibilities
  • Conduct individualized clinical assessments of patient records to ensure safe transitions of care.
  • Communicate with referral sources to gather essential information for continuity of care.
  • Utilize clinical nursing skills to identify and verify patient appropriateness for home health level of care.
  • Provide education and support to patients, families, and referral resources throughout the home health referral process.
  • Collaborate with the clinical team to coordinate home health referrals and meet financial targets.
  • Identify opportunities for other services within the IDN and facilitate referrals.
  • Maintain current knowledge of COPs, ACHA guidelines, The Joint Commission standards, and CMS payor models.
Requirements
  • Associate Degree in Nursing
  • Minimum two years of acute care experience as an RN
  • Current, valid State of Florida RN license or endorsement
  • American Heart Association Basic Life Support Healthcare Provider Card
Preferred Qualifications
  • Bachelor Degree in Nursing
  • Minimum one year home health nursing or case management experience
Physical Requirements
  • Sedentary work with occasional walking, bending, and stooping
  • Long periods of computer time or at workstation
  • Light work with lifting or moving objects up to 20 pounds
