Licensed Vocational Nurse

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our Home Health team at Sutter Health. As an LVN, you will provide high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.
Job Summary:The LVN will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing direct patient care under the supervision of a registered nurse or physician. You will also be responsible for educating patients and their families on disease management, medication administration, and other health-related topics.
Responsibilities:- Assess patient needs and develop individualized care plans
- Provide direct patient care, including medication administration, wound care, and vital sign monitoring
- Educate patients and their families on disease management, medication administration, and other health-related topics
- Collaborate with the interdisciplinary team to coordinate patient care
- Document patient progress and outcomes in the electronic medical record
- Graduate of an accredited school of vocational nursing
- Current LVN licensure in the state of California
- Basic Life Support (BLS) certification
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
