Responsibilities:
- Provide nursing care to patients in their homes, under the supervision of a registered nurse or physician.
- Assess patient needs and develop individualized care plans to meet those needs.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and report any changes or concerns to the healthcare team.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Document patient information and progress in a timely and accurate manner.
Requirements:
- Graduate of an accredited school of vocational nursing.
- Current LVN licensure in the state of practice.
- Basic Life Support (BLS) certification.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
Work Environment:
This position requires frequent travel to patient homes, with occasional travel to our office or other healthcare facilities. The ideal candidate will have a valid driver's license and reliable transportation.
