Key Responsibilities- Provide nursing care: Under the direct supervision of a registered nurse or physician, you will provide nursing care to patients in their homes, adhering to established standards of care, policies, and procedures.
- Collaborate with healthcare team: You will work collaboratively with our healthcare team to develop and implement patient care plans, ensuring seamless communication and coordination of care.
- Support patient education: You will educate patients and their families on disease management, medication administration, and other health-related topics, promoting patient empowerment and self-care.
- Document patient information: You will accurately document patient information, including medical history, treatment plans, and progress notes, maintaining confidentiality and adhering to HIPAA guidelines.
- Participate in quality improvement: You will contribute to quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care and outcomes.
- Education: Graduate of an accredited school of Vocational Nursing.
- Certifications: LVN-Licensed Vocational Nurse, BLS-Basic Life Support Healthcare Provider, Blood draw certificate within 30 days.
- Experience: 1 year recent relevant experience.
- Skills: Demonstrated knowledge of basic nursing skills, various medical procedures, views, and equipment; ability to interpret a variety of data and instructions; basic understanding of human anatomy, physiology, pathology, and medical terminology; written and verbal communications skills; well-developed time management and organizational skills.
Work Schedule: Full-time, days, with every other weekend.
Work Environment: Home health setting, with occasional travel to patient homes.
Physical Demands: Ability to lift, bend, and stand for extended periods, with occasional exposure to infectious diseases.
