Responsibilities- Provide one-on-one care to clients, adhering to their individualized plans of care and promoting their dignity and independence.
- Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and assess the results of these interventions.
- Collaborate with other healthcare professionals, such as physical therapists, occupational therapists, and speech therapists, to ensure comprehensive care.
- Implement and maintain infection control policies, reporting any possible communicable diseases or infections.
- Accurately document nursing actions, communicate with family members, case managers, and physicians, and document progress and outcomes for established goals.
- Order supplies, medications, and feedings as needed to ensure adequate stock levels.
- Ensure all patient needs are met in accordance with physician instructions.
- Monitor incidents involving individuals, complete incident reports, and inform case managers.
- Maintain and report faulty medical equipment to the Durable Medical Equipment department.
- Current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
- Current Cardiopulmonary resuscitation certification.
- Ability to work in a home-like setting.
- Physical ability to bend, stoop, push, pull, sit, and walk for periods of time.
- Able to utilize proper body mechanics while lifting 50 pounds.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
