About the Role

We are seeking a skilled RN Charge – Transitional Care Specialist to join our team at Mother Joseph Care Center. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients in need.

Key Responsibilities
  • Lead and manage a team of Registered Nurses to deliver exceptional patient care
  • Develop and implement effective care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Monitor and evaluate patient outcomes to identify areas for improvement
  • Provide education and training to nursing staff to enhance their skills and knowledge
Requirements
  • Registered Nurse licensure in the state of Washington
  • 3 years of RN experience, preferably in sub-acute care, or previous charge nurse experience, or 2 years relevant leadership experience
  • National Provider BLS - American Heart Association certification
