Job Title: Physical Therapist Home Health

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in North Kansas City, Missouri.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist Home Health, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote optimal patient outcomes.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough assessments to identify patient needs and develop effective plans of care.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes accurately and in a timely manner.
  • Stay up-to-date with industry developments and best practices to provide evidence-based care.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • No experience required, but a passion for patient care and a commitment to excellence are essential.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
