Job Title: Home Health Physical Therapist

Alternate Solutions Health Network LLC is seeking a skilled Home Health Physical Therapist to join our team in Kansas City, Missouri.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote optimal patient outcomes.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough assessments and develop individualized plans of care to address patient needs and goals.
  • Implement and evaluate treatment plans, making adjustments as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Supervise and educate Physical Therapist Assistants and other healthcare staff as needed.
Requirements:
  • Current license to practice as a Physical Therapist in the state of employment.
  • No experience required, but a strong desire to work in home health and provide high-quality patient care.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
