Job Title: Physical Therapist in Home Health Setting

Job Summary:
Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in Kansas City, Missouri. As a Physical Therapist in Home Health Setting, you will provide high-quality care to patients in the comfort of their own homes.

Responsibilities:
• Provide skilled physical therapy services to patients in their homes as ordered by the attending physician
• Conduct thorough assessments and develop individualized plans of care to meet the unique needs of each patient
• Collaborate with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes
• Educate patients and their families on proper use of equipment and techniques to promote independence and safety
• Document all aspects of treatment, assessments, and patient education in accordance with agency protocol and Medicare/Federal guidelines

Requirements:
• Current license to practice as a Physical Therapist in the state of employment
• Minimum of one year of experience as a Physical Therapist in an acute setting
• Home care experience is desired
• Ability to effectively communicate and create positive impressions with patients, families, physicians, and co-workers
• Knowledgeable on therapy best practices
• Ability to make appropriate physical therapy judgments
