Job Title: Physical Therapist Home Health

We are seeking a skilled Physical Therapist to join our Home Health team at Alternate Solutions Health Network LLC. As a Physical Therapist, you will play a vital role in providing high-quality patient care and helping patients achieve their rehabilitation goals.

About the Role:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Conduct thorough assessments and develop individualized treatment plans to address patients' physical therapy needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes.
  • Document patient progress and maintain accurate records, adhering to agency protocols and Medicare/Federal guidelines.
Requirements:
  • Current licensure as a Physical Therapist in the state of Missouri.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills.
