Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at Innovative Renal Care. As a Clinical Social Worker, you will play a vital role in providing exceptional patient care and support to individuals with End Stage Renal Disease.

Key Responsibilities:
  • Assessment and Education: Conduct thorough assessments of patients' physical, emotional, and financial needs, and provide education on disease management and treatment options.
  • Care Planning: Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans that address patients' unique needs and goals.
  • Communication and Interpersonal Skills: Demonstrate excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Requirements: Master's degree from an accredited school of social work (MSW), current state licensure (e.g. LCSW, LMSW), and two years of clinical work experience, preferably in an ESRD setting.
