About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Agape Community Health Center. As a key member of our Behavioral Health department, you will play a vital role in providing high-quality therapeutic services to our patients.

Key Responsibilities
  • Provide direct services, including assessments, treatment plans, and discharge planning, to patients and their families.
  • Address referrals from providers, including warm hand-offs, and participate in daily huddles to assess needs and coordinate services.
  • Maintain weekly scheduled appointments in our electronic records systems and provide therapeutic services to patients and their families.
  • Work collaboratively with multidisciplinary team members, including external partners, to ensure comprehensive care.
  • Complete required documentation and paperwork, including biopsychosocial assessments and treatment planning, in a timely manner.
  • Participate in our SBIRT program and demonstrate cultural competency and commitment to patients.
  • Maintain a caseload of 15 to 20 patients weekly, with a minimum of 5 patients per day, and perform other duties as assigned.
Requirements
  • Licensed Clinical Social Worker (LCSW) in good standing in the State of Florida.
  • Bilingual capacity with the ability to provide services in English and Spanish (preferred).
  • Working knowledge of Clinic policies and procedures.
  • Experience supervising unlicensed staff and knowledge of community and community resources.
  • Ability to pass a background check and possess a valid Florida State Driver's License.
  • TB Clearance and flexibility to work overtime, holidays, and weekends.
  • Local travel required.
Preferred Qualifications
  • Bilingual capacity with the ability to provide services in English and Spanish.
  • Knowledge and experience working with co-occurring populations, with an emphasis on children and adolescents.
  • Strong clerical/administrative skills, including personal computer skills (Microsoft Office software).
  • Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures.
  • Demonstrated oral/written skills and ability to work effectively with people of diverse cultures, ages, and economic backgrounds.
