Job Summary

The Care Plan Coordinator is a critical member of our interdisciplinary team, responsible for ensuring the timely and accurate completion of the MDS, assessment protocols, and comprehensive assessments. This role requires strong leadership and communication skills, as well as the ability to work collaboratively with healthcare professionals to develop and implement individualized care plans.

Key Responsibilities
  • Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines.
  • Develop and implement comprehensive care plans that address the medical, nursing, and psychosocial needs of residents.
  • Collaborate with the interdisciplinary care plan team to ensure that care is provided in accordance with the resident's wishes.
  • Monitor and record the progress of each resident, making adjustments to the care plan as needed.
  • Participate in facility surveys and maintain accurate records of resident assessments and care plans.
Requirements
  • Current license in good standing in the state in which the facility is located as a Licensed Practical Nurse or Registered Nurse.
  • Ability to read and follow written directions, as well as communicate effectively with residents, family members, and healthcare professionals.
  • Strong leadership and supervisory skills, with the ability to work harmoniously with professional and non-professional personnel.
  • Ability to maintain confidentiality and handle sensitive resident information with discretion.
