**Job Summary:**

As a Hospice Care Coordinator at Henry Ford Health, you will play a vital role in providing comprehensive care to patients and their families in the hospice setting. Your expertise in assessing patient needs and coordinating with the interdisciplinary team will ensure seamless care delivery.

**Key Responsibilities:**

  • Assess patient needs and develop individualized care plans in accordance with the hospice concept of care.
  • Collaborate with the interdisciplinary team to provide complete patient/family care and referral services.
  • Provide pain management and symptom control to patients, ensuring their comfort and well-being.
  • Maintain continuity of care by ensuring patient needs are met and addressed in a timely manner.

**Requirements:**

  • Associate's Degree or Diploma in Nursing.
  • Understanding and acceptance of the Hospice concept of care.
  • Working knowledge of current pain and symptom management techniques.
  • Excellent interpersonal skills to present a positive image of hospice to patients and families.
  • Ability to adapt policies and procedures to meet the unique needs of hospice patients and families.
  • Previous hospice experience preferred.

**Certifications/Licenses:**

  • Current licensure in the State of Michigan as a Registered Nurse.
  • Hospice certification preferred.
