LifeBridge Community Services is seeking a contract registered dietitian to provide dietary services to our Sr. Meals on Wheels participants in the Greater New Haven region.
Job Summary:The successful candidate will be responsible for providing nutritional assessments, nutrition counseling, and developing an annual nutrition education plan. They will also conduct nutrition education sessions at senior community centers and prepare handouts as needed.
Responsibilities:- Conduct site visits with site directors to assess nutritional needs
- Provide nutritional assessments and counseling to home-delivered participants
- Develop and submit an annual nutrition education plan
- Conduct nutrition education sessions at senior community centers
- Prepare handouts and materials as needed
- Attend quarterly menu meetings
- Perform other duties as requested by the Director of Sr. Meals on Wheels
- Dietitian: 1 year of experience required
- Registered Dietitian license
The contractor will sign a confidentiality agreement and will be required to maintain confidentiality of client information.
Physical Requirements:The contractor will be required to travel locally by vehicle and interact with clients. They must be able to sit frequently and talk and hear. They must also be able to walk, use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; stoop, crouch, and kneel.
