Join Our Team as a Hospice Admissions Nurse

We are seeking a compassionate and skilled Hospice Admissions Nurse to join our team at Assured Hospice of Port Angeles. As a Hospice Admissions Nurse, you will play a vital role in ensuring that patients and their families receive the highest quality care and support during this challenging time.

About the Role

The Admissions Nurse is responsible for executing the hospice admission process in compliance with federal and state regulations. This includes advocating for the patient and family to meet their stated needs, assessing eligibility, and referring them to other resources as appropriate.

Responsibilities
  • Develop trusting relationships with patients and their families as an end-of-life care expert
  • Be valued and respected by patients and their families
  • Participate in employee-focused wellness and support programs
  • Enjoy incredible team support and empathetic leadership
Requirements
  • Must be a licensed registered nurse in the state of operation
  • At least 2 years of experience in a hospice, home health, or other healthcare service delivery system setting
  • Current CPR certification
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation
