Care Transition Specialist

We are seeking a skilled Transition Coach to join our team at The Mental Health Center of Greater Manchester Inc. As a Transition Coach, you will provide community-based interventions to individuals with behavioral health challenges, utilizing the Critical Time Intervention (CTI) evidence-based practice.
Key Responsibilities- Develop and implement individualized transition plans for clients.
- Provide CTI interventions, including coordination and monitoring of transition plans.
- Utilize a person-centered approach to develop, implement, and evaluate goals and plans with clients.
- Collaborate with community agencies and natural support systems to ensure coordination of services.
- Document service provision and maintain accurate records.
- Bachelor's degree in a related human service field or equivalent experience.
- At least 2 years of experience working with behavioral health populations.
- Additional certifications or credentials are a plus.
