Job Description:

VNA Health is seeking a compassionate Registered Nurse Case Manager to join our Home Health care team. The successful candidate will plan, organize, and direct home care services, ensuring the highest level of patient care and satisfaction.

Responsibilities:

* Conduct continuous review of patient caseload to ensure appropriate utilization of services and continued skilled need
* Monitor homebound status and review documentation in the medical record
* Maintain interdisciplinary communication and discharge planning
* Complete initial assessment of patient and family to determine home care needs
* Develop and implement a comprehensive care plan, incorporating therapeutic, preventive, and rehabilitative nursing actions
* Administer medications and treatments as prescribed by the physician
* Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan
* Counsel the patient and family in meeting nursing and related needs
* Knowledge of state, federal, local, and accreditation regulations pertaining to the delivery of home health services

Qualifications:

* Current and active California RN license
* Bachelor's degree, preferred
* Minimum one year acute care experience required
* One year recent home health experience preferred
* Current CPR certification
* Licensed driver with insured automobile - in accordance with state and/or organization requirements

Physical Requirements:

* Ability to lift 50 lbs
* Ability to push/pull 50 lbs
* Ability to stoop, bend, and squat
* Ability to walk up one flight of stairs
* Ability to walk one mile
* Ability to sit for one hour, minimum
* Visual and audio acuity
